Please ask your department/program if this is a requirement.
TO RECEIVE A RADIATION MONITORING BADGE (DOSIMETER) – IF REQUIRED:
- Complete the Dosimeter Request Form.
- Email the completed form to Radiation Safety at Radiation_Safety@lhsc.on.ca from your hospital email account.
- If you do not have a radiation dosimeter available, you will be required to step out of the room when x-rays are present.
Radiation Safety will issue the dosimeter to the appropriate badge board in advance of the program or rotation that requires it. Please ensure all eLearning is up to date as outlined within your ME(MyEducation) account.
NOTE:
- If you do not have a SIN number contact Radiation Safety via email at Radiation_Safety@lhsc.on.ca.
- If you are pregnant and require a Fetal Dosimeter please contact Radiation safety via email at Radiation_Safety@lhsc.on.ca.
- Dosimeters are not to leave the hospital. Please leave on the OR rack.
