How to refer patients to our program
For paediatric patients:
There is no specific referral form needed. Please fax a letter of referral to 519-667-6849.
For adult patients
Please use our Hematology Referral Form and fax it to 519-685-8294.

SCD Education Program for Health-care Professionals
The Sickle Cell Awareness Group of Ontario’s (SCAGO) SCD education program offers 13 modules, each with multiple micro-lessons under ten minutes in length. This program provides equitable, high-quality education for health-care providers, supporting the needs of people with SCD.
