At the Regional Stroke Centre, we are dedicated to ongoing improvement of our stroke services and value the input of persons and families with lived experience. This survey will take less than five minutes and is used to help continue to improve our stroke services and offer the best care we possibly can. Please only fill out this survey if you or your family member were a stroke patient admitted to the 7th floor at University Hospital.
Note - Survey participation is voluntary. This survey is intended to be anonymous, but there is a possibility that a respondent and/or patient could be identifiable depending on the answers provided to questions 4 and 5. Feel free to change the wording as you see fit.
