How Patients are Notified of Their Appointment
- Once the EMG Lab office has received the requisition, an appointment time and date will be booked for your patient.
- Our secretary will then fax a form back to your office with the patient's name, appointment date and time. The form also contains a map of the location of University Hospital and instructions that the patient must call our lab to confirm their appointment time.
- It is the responsibility of the referring physician's office to inform the patient of their appointment date and time. This form should be given to your patient with instructions to bring the form with them to the appointment.
