phytonadione (Vitamin K1)

Indication

  • Prophylaxis and treatment of vitamin K deficiency bleeding

Dosage Guidelines

  • Prophylaxis: Immediately after birth

    Less than or equal to 26 weeks gestation: 0.5 mg IV

    Less than or equal to 1500 grams and greater than 26 weeks gestation: 0.5 mg IM

    Greater than 1500 grams: 1 mg IM

  • Treatment: 1 mg IM/IV

Administration

  • IM
  • IV: Dilute to 1 mg/mL for infusion over 15 to 30 minutes in D5W or NS.  Protect from light and administer immediately after dilution.

Adverse Effects

  • Rarely: hemolytic anemia, hyperbilirubinemia, and hypersensitivity or anaphylactoid reaction following IV administration

Comments

  • After parenteral administration, blood coagulation factors increase within 1 to 2 hours

  • 2 mg/mL injection (0.5 mL ampoule)
