The Paediatric Complex Care program at Children’s Hospital is part of the Complex Care for Kids Ontario (CCKO) initiative, led by the Provincial Council for Maternal and Child Health. We serve as one of four regional CCKO hubs in the province.
Our dedicated team includes paediatricians, nurse practitioners, registered nurses, social workers, pharmacists and registered dieticians. We take a patient and family-centered approach to ensuring each child’s unique needs are met through the coordination of care, support navigating the health-care system and connection to valuable resources.
We aim to:
- Foster a supportive and well-coordinated care environment for children and youth with chronic and complex health needs who are considered medically fragile or technology dependent
- Reduce preventable hospital visits and the amount of time families spend trying to communicate with the right person
- Improve each patient and family’s overall experience with the health-care system
