Shared Decision Making is the process whereby health professionals and patients collaborate on medical decision-making with a more structured process that considers both the best evidence and patient values.
Supported by a Decision Coach outside of the traditional circle of care, the process helps:
- Both people receiving and delivering care understand what's important to the other person.
- People feel supported and empowered to make informed choices and reach a shared decision about care.
- Health and social care professionals tailor the care or treatment to the needs of the individual.
