Transitions in Care

The Transitions in Care team supports paediatric patients and their families across key transition milestones. It is home to a multidisciplinary team made up of Transition Navigators, Educational Liaisons, Child Life Specialists and Indigenous Wellness Consultants that work together to ensure child's individual needs are met within the health-care and education systems.

Transitioning from youth to adult care

Transitions in Care staff help patients as young as 12 with chronic or complex medical conditions prepare for their eventual transition to adult care at age 18. They do this by working alongside the patient's care team to assess transition readiness, develop individualized plans, and empower them to take an active role in their health-care journey.

A referral from your child's Children's Hospital care team is required.

Educational support for children and youth

Transitions in Care staff guide patients through educational transitions, such as moving from school to hospital and back to school again, and helps families advocate for their child's needs while in school. They do this by providing information about rights to education and types of accommodations available, often liaising directly with schools to make each child's transition as seamless as possible.
