We think about your discharge from the hospital as soon as you are admitted! The admitting nurse will ask you questions about how you manage at home, or if there is anything we need to consider before you go home. For example, how many stairs do you have, are you caring for someone and are you concerned about being able to care for him/her when you go home.
Most people follow a planned course of events and so their discharge date can be estimated fairly well. Occasionally the length of stay might be longer or shorter than first thought. Regardless, the team should be keeping you informed of the day you will probably be discharged. In some areas of cardiac care, a note is posted in the patient's room about the possible discharge date. This is to ensure that all members of the health care team, including you and your family, are preparing for that day.
Discharge time is approximately 11:00 a.m. Please arrange for transportation home as soon as you are able to.
If, for medical reasons, you are unable to return to your home and require long term care, our social worker and Southwest LHIN Case Manager will be helping to coordinate this with you and your family.
Prior to leaving the hospital, you will be given prescriptions, a follow-up appointment card (if required) and post-discharge information. Please ensure that you understand all the instructions you have been given. We want to ensure your recovery goes well!
