If you are a physician and feel that your patient may benefit from an additional assessment, you may refer your patient for a consultation with one of our physicians. Below are some general guidelines on how to make a consult referral.
Referral Guidelines
Cardiology Consults
- Fax consults to physician's office.
- Include cover sheet, reason for referral, current history, medications, test results and recent bloodwork.
Arrhythmia Service Consults
- Fax consults to Arrhythmia Service at 519-663-3782.
- Include cover sheet, reason for referral, current history, medications, test results, ECGs, and tracings of the arrhythmia.
Cardiac Surgery Consults
- Fax consults to Surgeon's Office.
- Include reason for referral, current history, medications, test results, and bloodwork.
