After the birth of your baby, you will receive a document, "Statement of Live Birth", which must be filled out and sent or taken to the City Clerk's Office within 30 days of your child's birth. You need to do this to make sure your child is registered and so that you can purchase your child's Birth Certificate if you choose to do so.
