How Your Appointment is Scheduled
- You must be referred to the MND Clinic by a medical doctor such as your family physician or a specialist with a medical degree such as a neurologist. You cannot book an initial appointment with us yourself.
- After your physician contacts us with the referral, we will send the appointment details back to him/her. Your physician will then be responsible for notifying you of your appointment date and time.
- If for any reason you will not be able to keep your appointment with the team, please notify the appropriate physician's office: Dr. Shoesmith's office at (519) 663-3597, or Dr. Strong's office at (519) 663-3874.
- Once you have been seen at our MND Clinic, all follow-up appointments are mailed to you after consultation with the Neurologist as to the frequency for your particular needs.
