Indigenous Transitions Facilitator – Regional Renal Program
The Southwest Regional Renal Program is taking active steps to advance more culturally safe, inclusive and equitable health care with Indigenous Peoples. We are committed to walking the path of reconciliation, to listening and learning from the Indigenous communities we serve, and answering the Truth and Reconciliation Commission's Calls to Action related to health care.
The Indigenous Transitions Facilitator is available to help support the unique needs of Indigenous patients undergoing renal treatment.
Some of the ways the Indigenous Transitions Facilitator can help are:
- To lead Indigenous engagement within the renal program.
- To develop kidney screening and disease prevention programs unique to First Nation, Inuit, Metis and Urban Indigenous population’s needs.
- To provide information and education in a culturally safe manner.
- To liaise and advocate between patient, families and the care team.
- To provide consultation to clinical teams with a cultural and trauma informed lens.
