Oncology Patient Navigation Program

- Step 1: Meeting criteria testing complete with suspicion of cancer.
- Step 2: Referral to OPNP can be sent.
- Step 3: Referral received and triaged.
- Step 4: Registered with OPNP and sent to next available MRP.
- Step 5: Appropriate:
- Note added to chart and referral sent back to referring provider. OPNP will close the chart.
- Step 6: OPNP will contact patient directly via telephone.
- Step 7: OPNP will navigate patient through staging as ordered by MRP.
- Step 8: OPNP will become point of contact for patient until surgery or Cancer Centre.
- Step 9: OPNP to close if:
- Surgery complete.
- Patient is treated at Cancer Centre.
