Medication Reconciliation, sometimes called "Med Rec" is a term used to describe the process of comparing a patient's medication and drug dose history to his or her current medications. This is done to prevent accidental disruption of important medications or drug dose errors.
Medication Reconciliation is done each time a patient is admitted to hospital, moves from one ward to another, or is discharged to the community. When patients are unable to speak on their own behalf, we need the help of the patient's family, family physician and community pharmacist.
Upon admission, each patient's family will be asked to provide a list of the patients medications and allergies, in order to complete a medication history. We may also need to contact the patient's family physician and pharmacy to ensure the most accurate information.
